Voluntary Participation – I understand my participation is completely voluntary and I can refuse to participate or I can withdraw from the panel or (any of) the Research at any time without penalty or prejudice. If I choose not to participate in one Research, it will not affect my ability to participate in future studies.

Confidentiality/Intellectual Property -As my participation in the panel and the related Research may involve disclosure and/or development of confidential products, packages, pictures of products, usage instructions, descriptions of products, and/or ideas and environments, which, separately or together with any (new) brand(s) or brand concept(s), may be patentable or otherwise protectable by the Researcher and/or third party partner/affiliates of the Researcher, I agree to preserve the confidentiality of this testing and any confidential information in any form, to which I am exposed during my participation in the panel and related Research, directly or indirectly. I agree that I will neither use nor discuss the above information with anyone, not even with my family and friends, except the person administering the test. I also agree that my obligations of confidentiality and non-use, extend beyond my participation in the panel and related Research.  I voluntarily agree to assign to the Researcher and/or any third-party partner/affiliates, all ideas, developments, discoveries, works of authorship, designs, software, and/or inventions, patentable or not, conceived by me as a result of my participation in the panel and related Research. Furthermore, I voluntarily agree that the Researcher and/or any third-party partner/affiliates may use these ideas, developments, discoveries, works of authorship, designs, software, and/or inventions in any manner it sees fit.  My obligations under this agreement will not extend to any products or other information that are generally known to the public or which become available to the public through no fault of mine.

Responsibilities - I understand that, during my participation in the panel and related Research, I am expected to follow the study directions that are provided to me and conduct the study exactly as directed. I understand I may be directed to stop participating in the panel and related Research if I do not follow these directions. All test products remain the property of the Researcher and/or any third-party partner/affiliates. I agree not to remove or use any products from the test or site without the express permission of the Research owner and/or any third-party partner/affiliates who may be conducting the Research. I agree to return all unused test products (even empty packages) and/or used appliances at the end of the test period, including any other (empty) products/packages/prototypes that have been provided to me for comparison with the test product.

Risks and Benefits – The risks associated with participation in Research including product usage are comparable to those associated with the use of marketed products similar to the one(s) being evaluated in Research.

Contact Information – When participating in a particular Research including product usage, should you experience adverse health problems that you believe may be caused by any of the products, please stop using the product(s) and contact your personal physician if not urgent or contact an emergency medical facility if urgent and/or life threatening.  All incidents should be reported to the research facility and/or study owner (via contact details as found on the product label or the placement letter).  If you have any other non-safety related problems or questions during any of the studies, please reach out to the study owner. 

Photo/Video/Audio Release & Use - I understand that as part of my participation in the panel and related Research, certain portions of the Research may be audio and/or videotaped or I might be photographed during the Research. Any recordings in which I appear and other information I provide (e.g. text, video, images or audio) (“Research Data”) may be used by the Researcher and/or any third party partners/affiliates for market research, trade presentations, product innovation, and internal training as determined by the Researcher and/or any third-party partners/affiliates, in whole or in part and can be shown to business partners that have an interest in the results of the studies. 

Data Privacy - I understand that if any of my personal information is collected, it will be used for Research and may be used for communication with third-party business partners. I also understand the Researcher is the controller of such data and will take all reasonable steps to ensure my personal information is protected. I agree that my personal information can be shared with the Researcher’s third-party partners/affiliates for the purpose of enabling set-up and communication regarding Research for which I am selected and have data output of the Research available (incl. photo/video/audio, not identified by name).  I will not be identified by name in any publication regarding the Research.

If I am a resident of the EU-UK, I have the right to access the personal data Researcher holds about me and I can exercise my data protection rights, which may include accessing, correcting, erasing, or holding processing of personal data, or withdrawing consent to use my personal data, by contacting the Researcher at Humaninsightsplace.com and referencing the UK Testerclub.  My personal information may be transferred to countries outside the region in which I live, including the US, but my information will still enjoy adequate data protection and safeguards as may be required by the EU-UK laws. Liability - To the extent permissible by law, the Researcher, its employees and agents and any 3rd party partners/affiliates, will not be responsible or liable for any loss, claim, damage of any kind, whether based in contract, tort or otherwise, that arises out of or is in any way connected with my participation in this Research, including but not limited to my use of the test products and my travel to and from, and visit of, the Researcher’s facility or location where the Research is taking place.

Expense – I understand no payment is required to participate in the panel and related Research.

Compensation – Unless stipulated otherwise, no compensation will be provided for your participation in the panel and related Research, except the opportunity to use product consistent with the study directions.

I confirm that I am at least the age of majority in my country and can validly consent to be a member of the panel. I have read the Informed Consent and I have had the opportunity to ask questions about this panel subscription and those have been answered to my satisfaction.

I voluntarily agree to join the UK Testerclub Panel and I consent to the Researcher, its affiliates and/or third party partners, processing my personal information in accordance with this document. I acknowledge and explicitly agree that the personal data processed for the purpose of performing the Research (studies) may include processing personal data concerning my health and/or my racial and ethnic origin. I understand I can ask to be removed from the Panel at any time.
